How to Enroll
APPLICATIONS FOR THE 2010-2011 SCHOOL YEAR
ARE NOW BEING ACCEPTED
 
Pick up application during normal school hours (7 a.m. to 4 p.m.) or download and print from this web site.
 
Complete all parts of the application.  Return the application to Middle College High School in person.
 
Student MUST reside in the Los Angeles Unified School District Boundaries.
 
Attach the following documents to the application:
  • Recommendation Forms (in the application) must be returned in sealed envelopes.
  • Copy of ID20 (LAUSD applicants)
  • Copy AT10 (LAUSD applicants)
  • Official school transcript (Private/charter/non-LAUSD applicants)
  • Attendance history (Private/charter/non-LAUSD applicants)
  • Typed Essay (topic provided in the application)
  • Copy of final fall semester report card
